What is inside?

  1. 1. Where to startIdentify the billing department and ask about help before choosing how to pay.
  2. 2. Words for the callUse a short phone script and ask about the application, deadline, and billing hold.
  3. 3. Documents to organizeKeep track of what the hospital actually requests and how it wants you to submit it.
  4. 4. Follow-up and next stepsRecord the response, confirm receipt, and know what to ask if the answer is no.

Use it in three short steps

  1. Open the PDF and print a clean copy, or save it on a private device.
  2. Call a verified number on the hospital’s bill or official website. Keep the checklist beside you.
  3. Record the instructions privately. Ask when to follow up and save proof of anything you submit.

First sentence to try: “I cannot afford this bill. Do you offer financial assistance or a discount?”

The hospital decides eligibility under its own policy. This checklist is an organizing tool, not an application, an eligibility decision, or a promise to reduce a bill. It does not replace the hospital’s forms.

Choose the guide that fits your situation

Keep the completed copy private.

Do not upload or email bills, applications, income documents, account numbers, or a completed checklist to us. Send an application only through the hospital’s approved method. Do not delay emergency care to complete paperwork.

Can I share this?

Yes. Share this page or an uncompleted copy with someone who may find it useful. Libraries and community organizations may offer the free checklist with attribution to The Health Question. Do not imply that we are a hospital, a government agency, or an application-processing service.

Where the guidance starts

Our companion guides link to CMS guidance on financial assistance and the IRS requirements for tax-exempt hospital policies. Requirements depend on the hospital and circumstances. Learn how we prepare and check our guides.
